A digital place for people who access services, carers, staff and partner agencies, to share ideas around care co-ordination & care planning in mental health

About Donna Kemp

My background is mental health nursing, I have worked on acute admission wards, crisis team and assertive outreach; I have also undertaken a lecturer practitioner role at Huddersfield University.  My current role is Care Programme Approach Development Manager at LYPFT. On a personal note, I am a married mum of 3; I enjoy spending my time with family and friends.  I would love to list lots of super interesting hobbies but in reality I don’t have time for any.

  1. You may wish to look at http://www.infostandards.org/careplanning for some national guidance.

    • Thanks for posting Zac; I would be interested in how this works in mental health

      • There are some generic bundles published for basic mental health requirements, however we are aware there is some development to support this requirement in full. When we have a partner to work with that requires mental health content, we should be able to take this further. If you use the contact us on the site, we can talk about how the content can be used or developed further.
